Training Services Agreement

The parties agree as follows:

Services: Trainer agrees to provide private lessons for Client and Dog on a lesson-by-lesson basis, the goal being to teach Client how to train and work with the Dog. These lessons will primarily take place at Client's home. Client understands that he/she and members of the household must follow Trainer’s instructions without modification, work with dog daily as recommended, and constantly reinforce training being given to the Dog.

Cancellation Policy: If Client fails to give at least 24 hours cancellation notice, or is not present at time of scheduled appointment, session fees are still due. For a package deal, the session will still be counted as one session. Please see our Policies page on the website for extra details.

Liability: Dog Owner acknowledges that Affinity Dog has no control over the Dog when the Dog is not in the exclusive custody, care and control of Affinity Dog, and Owner agrees to indemnify and hold Affinity Dog harmless from and against any and all claims of injury, expense, costs, loss or damages caused by the actions of the Dog when the Dog is not the sole custody, care and control of Affinity Dog. Dog Owner acknowledges and agrees that owning any animal, including the Dog, has inherent risks, including but not limited to the risk of dog bites to Owner.

At Trainer's sole election, Trainer's duties shall terminate if (a) in Trainer's sole judgment Dog is to dangerous or vicious to Trainer or any other person or animal, or interferes with the training of other dogs, or (b) Client breaches any term or condition of this Agreement. Upon termination in accordance with the foregoing, Trainer's duties shall terminate but all other provisions of this Agreement shall continue in full force and effect.

This contract for Training Services supersedes all other agreements, written or oral, previously made between Client and Trainer.

I hereby assign and grant to Affinity Dog the right and permission to use, reproduce, distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) made of me and/or my dog(s) at any time during the training and/or boarding of my dog(s) by Affinity Dog, and I hereby release Affinity Dog from any and all liability resulting from such use, reproduction, distribution, and publication.

I hereby authorise the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) without limitation at the discretion of Affinity Dog and I specifically waive any right to any compensation I may have for any of the foregoing.
